Insurance: As of May 1, 2020, Nutrition and Wellness Consulting, LLC will no longer accept Insurance. A Superbill is provided to submit to insurance for Medical Nutrition Therapy (MNT) out-of-network coverage.
Payment: All payments will be sent via invoice. Visa, MasterCard, American Express, HSA accepted.

Missed appointments without notice will be charged a $25 fee
